AGE:              18 years or older

CITIZENSHIP:      Not required. If foreign born, must provide
                  proof of legal entry into the United States.

RESIDENCY:        No requirement

RECIPROCITY:      Yes EDUCATION AND     Graduate of School of Physical Therapy approved
   EXPERIENCE:    by American Physical Therapy Association (APTA).
                  Pass National Physical Therapy Examination
                  (NPTE) for Physical Therapist Assistants.
                  Be good moral character.
                  NOTE: For alternate requirements, consult
                  issuing agency.
