FAMILY GROUP DAY CARE PROVIDER

Provides care and/or treatment of nine to twelve children within
a 24-hour period in a family day care home. (See "Family Day
Child Care Provider" license on previous page for care of less
than nine children).

AGE:           21 years or older

CITIZENSHIP:   No requirement

RESIDENCY:     Required

RECIPROCITY:   NoEDUCATION AND  Hold at least an associate degree from an
  EXPERIENCE:    accredited program in child development or early
                 childhood education, or a Child Development
                 Associate certificate (CDA) in family day care.
               Submit detailed background information and medical
                 statement.
               Submit evidence of having been finger printed by
                 state or local police.
               Pass a Child Abuse and Neglected Tracking System
                 (CANTS) check.
               Home where day care is to be provided must be
                 inspected and be in compliance with the State
                 Fire Code for Group Day Care Homes.
               Provider and home must be in compliance with the
                 Family Group  Day Care Home Regulations for
                 licensure.
               NOTE: For alternate requirements, consult
