CITIZENSHIP:      Not required. If foreign born, must show proof
                  of legal entry into the United States. 

RESIDENCY:        No requirement

RECIPROCITY:      No EDUCATION AND     Be a physician or hold professional rank in a
EXPERIENCE:         college or university.
                  Must submit curriculum vitae and protocols for
                    research design or program.
