In a general sense, the rights of the pregnant woman always come first, but given that a surrogate mother unquestionably made the concious decision to become pregnant, and is being paid by anothe party to be pregnant and to eventually present them with a healthy child, I think she also voluntarily agrees to relinquish some of those basic rights. (And too, IRL, most surrogate arrangements include a psychological evaluation of the surrogate, which would surely include stuff like 'how do you feel about c-sections!)
