Let's just jump on that bad-fanfiction-wagon:

It was Lucy's dream she was having while in a coma after getting hit by a 
car that was aiming for her -  Carter pushed her out of the way. And while 
in the coma, she had his baby she didn't tell him about who had a rare 
disease that required a blood transfusion from its father that Abby 
discovered by accident *was* Carter, who she had been humping yet again. Add 
the prerequisate Carol and Doug still in Chicago, Weaver and Susan having an 
affair, and Luka pining for Carter while Romano is choosing between Mark and 
Neela.

That's the dream.
