Does this mean that an official mistake has actually been made?

In my outspoken opinion, two thirds of the discussion in this echo wouldn't be 
here had there been any acknowledgement that errors have occurred.

I've yet to see Foxy show any signs of being aware of the remotest possibility 
she could screw up.
