A good book is David Butenhof's "Programming with POSIX Threads".  I was
lobbying to use it as _the_ pthreads documentation for the operating
system that I developed a large chunk of the threads support on, but
that fell on licensing issues.

Another good reference of references is Bil Lewis' pages, the top
URL is http://lambdacs.com/threads/threads.html.
