The user has to set up port forwording for which ever application needs it. However, there are applications that can work without the user needing to forword his/her ports and that can be done with UPnP (universal plug n play) which I believe is a Microsoft technology, correct me if I am wrong, and it can be used for things like that. As to how to actually do it is still up in the air for me as I just started learning how to do socket programming two days ago. Good luck! .
