I doubt that different WinHelp.exe is used when calling the WinHelp() 
function and when using the explorer.

It might be something else, like not finding the neccessary files for 
the help (like .cnt), or not liking the window handle passed to the 
function.

Which compiler are you using? If there are examples with help
included with it, do they work?
