i seem to recall that microsoft systems journal (or MSDN magazine, as it 
is now called) did a whole series of articles on this issue, with each 
new item seeking to point out holes in the prior method and purporting 
to fix those holes.  my recollection is that it was in no way as simple 
as it used to be in the "old days" of 16-bit windows.  i forget when 
this series ran, but i'd guess at somewhere in 1999 or 2000 maybe.  you 
might try searching an index of the magazine if they have one online, or 
if you know someone with the archive CDs.
