If QA and management are not capable of doing QA and management
they should be replaced. I've done both and I guarantee you it
would be _extremely_ difficult for anyone to neglect their
responsibilities and pass off junk welds (or any other
potentially hazardous mistakes) if I was doing the
QA/management there.

I don't agree. Where I have worked people don't start out being
careless but they _do_ become careless if they think that their
best efforts won't be notice/appreciated by QA and management.

It can be taught, I've done it many times. Those who refuse to
change can be gotten rid of and replaced.
