Hi Mike,

It is plugged in at my end and can split an incoming line to three
devices.

It is named Versa-Link ATX-300 and made in USA by Multi-Link, Inc. at
1391 Leestown Road, Lexington, KY 40508.

I bought it 1992 and it has worked perfectly since then.
