The usual method is to dd the boot floppy to a disk image, and use that imaage as the -b parameter to mkisofs to combine the boot image with your windows CD files... then burn the output using cdrecord. Since you said you started with a disk image instead of a floppy, it seems like that would make it easier by removing a step. I still haven't figured out exactly what the difficult part is.
