If I remember correctly, the cd burning software I was using prompted me for a boot image to add to the CD. Otherwise, I just added files to the CDROM normally. After the CD had booted, I believe it actually used 2 drive letters in windows, one for the boot image, and another for remainder of the CDROM's contents.
