Nobody knows for sure.  There is a suggestion that it started when a 
well-known station operating around 1908 by Albert Hyman, Bob Almy and 
Peggie Murray used their names and then their initials (HAM) as a station 
identifier.  That did occur, but there is some evidence to suggest that 
the term was in use prior to then.
