I don't know how it works in Canada but people here who are on government assistance get their meds at a free or reduced price (some do) and the government does give subsidies to pharm companys for R&D, I'm sure it's nowhere near the amount it takes to actually do the R&D but for serious diseases etc where a cure is needed fairly quickly the company do get subsidies.
