You see, I don't see this with HMOs whatsoever. In fact, it's the opposite. One almost needs to have a limb hanging off before they can be referred beyond the primary care physician. If I wanted to have unnecessary surgery, I'd have to go "out of network" to find some doctor (which means paying on my own and not under my insurance). Making the medical profession both less lucrative and less attractive to true professionals will definitely have an impact on the quality of care, however.
