How are you defining patient autonomy?  If you are thinking in terms of  personal control or mastery, I have one suggestion:  The Pearlin-Schooler Mastery Scale.  It is only 7 items.   Also, Dr. Jochen Brandstadter of the University of Trier has developed a scale to measure two complementary modes of coping, assimilative tendency and accommodative tendency, both of which are positivey related to generalized internal control beliefs.
