There is plenty of literature on doctors becoming doctors, the role of nursing(is it a profession?)and the various other medical people who may be defined as the "fringe" of the medical delivery system by the MDs. Check out Andrew Abbotts work on psychiatric work, especially his notion of jurisdiction. Ed
